We are hiring Support Coordinators to train and grow with us! We are looking for reliable, customer service-oriented professionals to assist individuals and their families in the development of personalized service plans. Support Coordinators assist consumers by identifying and linking needed services and supports to improve quality of life within the home and the community. As a Support Coordinator, you must be willing and able to travel in Burlington, Camden, and Gloucester counties. Support Coordinators must have a flexible schedule to accommodate consumers schedules. This is an in-office position and not remote.

Full time positions available to teach life skills/employment skills to individuals with disabilities. Positions are office and field based. There is mandatory training, drug screening and background checks. You must possess a valid driver’s license and a clean driving record. First Aid and CPR certification is a major plus.

  • Full time: 35 hours a week
  • Dress code policy is business casual (very business casual)

Primary Job Responsibilities & Duties, but not limited to

  • Develops, implements, and monitors a personalized plan of care with input from the individual and family, based on the desired outcomes and personal preferences
  • Utilizes available resources in combination with formal and informal supports and services to meet the needs of the individual
  • Facilitates monthly, quarterly, and annual contact (via phone or face-to-face) with individuals and families to discuss progress toward achieving goals identified in the support plan
  • Ensures case management notes and progress reports are documented properly
  • Assists individuals and their families in coordinating, selecting, and taking ownership of their plan
  • Attends outreach, marketing opportunities
  • Position Requirements

 

Education & Experience

  • Bachelor’s Degree in a relevant discipline and one year of experience working with Adults with intellectual and/or developmental disabilities
  • Able to solve problems independently, effectively and creatively
  • Excellent time management skills to be able to juggle multiple plans for multiple families simultaneously
  • Must have a flexible schedule to meet the needs of our families. General hours are 8:30AM to 4:30PM or 9:00AM to 5:00PM with one-hour unpaid lunch.
  • Excellent customer service and communication skills
  • Must be able to thrive in a team environment
  • Must have a valid driver’s license and reliable transportation
  • Proficient in MS Office Outlook, Word, Excel, and PowerPoint
  • Caregiving: 1 year (Preferred)
