Supports Program Eligibility / Enrollment Services
To access services through DDD’s Supports Program an individual must:
- Be 21 years of age or older
- Be determined to be eligible for DDD services
- DDD Intake Application (see Apply for DDD Services)
- NJ CAT Assessment (see NJ CAT Resource Page)
- Be determined to be Medicaid eligible (see Medicaid Eligibility)
Once DDD eligibility (including Medicaid eligibility) has been determined, an individual must:
- Complete the Support Coordination Agency Selection Form and be assigned to a Support Coordination Agency
- Sign the Supports Program Participant Enrollment Agreement
- With the Support Coordinator, complete the Person-Centered Planning Tool (PCPT)
- With the Support Coordinator, complete the Individualized Service Plan (ISP)
- Be determined to be eligible for DDD services
- Be determined to be Medicaid eligible (see Medicaid Eligibility)
- Not be enrolled on any other Medicaid waiver (such as DDD’s Community Care Waiver)
- DDD Intake Application (see Apply for DDD Services)
- NJ CAT Assessment (see NJ CAT Resource Page)
- Supports Program ServicesThe services and supports available through enrollment in the Division’s Supports Program fall into one of the following categories:
- Employment/Day Services
- Individual/Family Supports
- Supported Employment
