Case Management Services
In addition to our core services, Resources for Independent Living provides case management services for individuals with disabilities for several programs. These programs include:
- Traumatic Brain Injury Fund for the NJ Division of Disability Services (NJDDS) for all seven southern counties
- The Supports Program for the NJ Division of Developmental Disabilities (NJDDD) for Burlington, Camden and Gloucester Counties
- Adult Protective Services (APS) for Cumberland County
- Statewide Respite Program for Cumberland County
- Housing First Homeless Program for Cumberland County

The TBI Fund provides New Jersey residents of any age, who have survived a traumatic brain injury, the opportunity to access the brain injury related services and supports they need to live in the community. The Fund purchases supports and services to foster independence and maximize quality of life when insurance, personal resources, and/or public programs are unavailable to meet those needs. A portion of the Fund also is used to support public education, outreach, and prevention activities related to TBI.
ELIGIBILITY
Fund recipients must:
- Provide medical documentation of brain injury
- Have liquid assets of less than $100,000
- Be a resident of New Jersey for at least 90 consecutive days
DEFINITIONS
Traumatic Brain Injury
To qualify for the Fund, an individual must have an acquired brain injury; defined as an injury to the brain caused by a blow or jolt to the head or a penetrating head injury/neuro-trauma that disrupts the normal brain function, where continued impairment can be demonstrated. This definition does not include dysfunction caused by congenital or degenerative disorders, birth trauma, or injuries caused by other circumstances.
Liquid Assets
Liquid assets include checking accounts, savings accounts, CDs, stocks and bonds. The Fund does not consider your primary home or your primary vehicle as “liquid assets,” nor do we consider your IRA or 401k, unless you are of retirement age.
Services and Supports
The Fund will consider supports and services that are related to your brain injury. These may include but are not limited to:
- Home modifications
- Service coordination
- Assistive technology
- Cognitive therapy
- Neuropsychological services
- Pharmaceuticals
- Physical, Occupational, and Speech Therapies
APPLICATION FOR THE TBI FUND
Contact DDS to request a TBI Fund Application at 1-888-285-3036.
Upon receiving your completed application and supporting medical documentation, TBI Fund staff will assign a case manager who, together with you, will develop a support plan to assist in determining your needs. Your support plan will be reviewed by TBI Fund Staff and the Review Committee. You will then receive a letter from DDS notifying you of the decision.
The Supports Program – Division of Developmental Disabilities
Resources for Independent Living provides Supports Coordination in Burlington, Camden and Gloucester Counties. We have been providing this service since the start of the program. We have experienced staff that are trained to meet the needs of individuals served on this dynamic program. We pride ourselves on providing quality supports to those we work with. Our case loads are kept low for each support coordinator so that we can focus on providing you excellent service. Our agency provides many services that are open to everyone. We are a community based provider who knows local resources so that we are able to connect you with additional services.
*Starting in the winter/spring of 2018 we will be providing direct services to individuals in Cumberland and Salem Counties.
Supports Program Eligibility / Enrollment
To access services through DDD’s Supports Program an individual must:
- Be 21 years of age or older
- Be determined to be eligible for DDD services
- DDD Intake Application (see Apply for DDD Services)
- NJ CAT Assessment (see NJ CAT Resource Page)
Be determined to be Medicaid eligible (see Medicaid Eligibility)
Once DDD eligibility (including Medciaid eligibility) has been determined, an individual must:
- Complete the Support Coordination Agency Selection Form and be assigned to a Support Coordination Agency
- Sign the Supports Program Participant Enrollment Agreement
- With the Support Coordinator, complete the Person-Centered Planning Tool (PCPT)
- With the Support Coordinator, complete the Individualized Service Plan (ISP)
- Be determined to be eligible for DDD services
- Be determined to be Medicaid eligible (see Medicaid Eligibility)
- Not be enrolled on any other Medicaid waiver (such as DDD’s Community Care Waiver)
- DDD Intake Application (see Apply for DDD Services)

Adult Protective Services – Cumberland County Only
Resources for Independent Living provides case management services for Cumberland County. We investigate reports of abuse, neglect and exploitation of adults who are not cognizant and aware of person, place or time. If someone you know is 18 years of age or older, living in the community and is the subject of abuse, neglect and/or exploitation, contact your county APS office. If you are reporting a case for Cumberland County please contact (856)825-0255.
An APS investigation is a thorough assessment of a potential at-risk adult. Within 72 hours of a referral’s receipt, a face-to-face meeting with the adult by a trained APS social worker is required. During this meeting the potential client is interviewed in private. Every effort is made to determine the competence of the adult. Other individuals or agencies that have knowledge of the situation may also be interviewed. When the investigation is complete a report is submitted to the APS Supervisor who will then consult with the social worker and determine if the adult is at risk of abuse, neglect or exploitation. All information generated by the investigation is confidential.

Statewide Respite (Cumberland County Only)
The NJ Statewide Respite Care Program has been operational since April of 1988. This program provides respite care services for elderly and functionally impaired persons age 18 and older to relieve their unpaid caregivers of stress arising from the responsibility of providing daily care. A secondary goal of the program is to provide the support necessary to help families avoid making nursing home placement of their relatives. Services available under the Statewide Respite Care Program include:
- Companions
- Homemaker/home health aides (on an hourly or overnight basis)
- Medical or social adult day care
- Temporary care in licensed medical facilities
- Camperships
- Private duty nursing service
- Caregiver-Directed option

Housing First Case Management – Cumberland County Only
The housing first program is a collaboration between Resources for Independent Living (case management), PRAC of Southern NJ (housing), Inspira Hospital, Gateway Community Action Partnership (lead agency), Cumberland County Jail, Complete Care (healthcare services), many churches, civic groups and the M25 Initiative. Through our dedicated partnership we have resolved to end homelessness in Cumberland County by 2020. We received 42 housing vouchers from the Department of Community Affairs to house the chronically homeless. Individuals must be chronically homeless for at least a year and have a disability to qualify for the program. Currently we have 42 individuals who are expected to receive vouchers and we are taking names only on a waitlist basis until we obtain additional vouchers.
